The Participant is the child whose name appears on the signature page. The Parent/Guardian is the adult signing on the Participant's behalf.
Parent/Guardian represents that they are the lawful parent or legal guardian of the Participant and have full authority to consent to the Participant's participation in academy activities.
Academy activities may include hiking, camping, archery, firecraft, outdoor cooking, fitness, rope climbing, navigation, service missions, and similar outdoor and physical activities. Such activities involve inherent risks including but not limited to falls, sprains, fractures, abrasions, lacerations, burns, allergic reactions, dehydration, heat or cold illness, animal/insect encounters, weather exposure, equipment failure, and unforeseeable accidents. Parent/Guardian acknowledges these risks on behalf of themselves and the Participant.
Activities involving archery, fire, blades, tools, ropes, and similar implements will be conducted under supervision and posted safety protocols. Parent/Guardian acknowledges the additional risk inherent in such activities and consents to Participant participation subject to age, training, and instructor approval.
Certain activities are designated 'high-risk' (e.g., supervised vehicle activities at legal/insured facilities, professional paintball facilities, legal/certified adult-supervised firearm or hunting education). Such activities require a separate signed special-consent form, additional eligibility criteria, and may not be available to all ages. This waiver does not authorize participation in high-risk activities absent a properly executed special-consent form.
In the event of a medical emergency, Parent/Guardian authorizes academy staff to call 911 or local emergency services, summon an ambulance, and consent to emergency medical evaluation and treatment of the Participant by licensed personnel. Parent/Guardian agrees to assume responsibility for any costs associated with such care.
Parent/Guardian permits academy staff to provide basic first aid within the staff's scope of training while awaiting professional care, including but not limited to bandaging, splinting, allergic-reaction support per the Participant's documented plan, and stabilization in line with the academy's published medical readiness scope.
Parent/Guardian represents that the medical information provided in the application is accurate and complete and will be updated promptly as conditions change.
Parent/Guardian is responsible for the Participant's behavior at academy events and for ensuring compliance with the academy's published code of conduct. Parent/Guardian agrees that disruptive or unsafe behavior may result in event removal at the academy's discretion.
All participants and adults agree to uphold respectful, modest, and safety-first conduct consistent with the academy's published standards. The academy reserves the right to remove participants who do not comply.
Parent/Guardian may consent (separately) to the use of photos and video of the Participant for academy purposes. The optional photo/video consent is recorded with the application.
To the fullest extent permitted by law, Parent/Guardian, on behalf of themselves and the Participant, releases and holds harmless Sons of Sunnah Academy, its founders, officers, instructors, volunteers, and partners from any and all claims arising from the Participant's participation in academy activities, except to the extent caused by gross negligence or willful misconduct. Parent/Guardian agrees to indemnify and defend Sons of Sunnah Academy from any third-party claims arising from the Participant's conduct at academy activities, to the extent permitted by law. (Template, pending counsel review.)
This waiver shall be governed by the laws of the state in which the academy is operated, without regard to conflict-of-laws principles. (Placeholder, confirm with counsel for the operational jurisdiction.)
If any provision of this waiver is found unenforceable, the remaining provisions shall remain in full force.
